All prayer requests are treated with care and confidentiality. They are shared only with our prayer team unless you indicate otherwise.

How to Set Up the Google Form

Go to forms.google.com and sign in with the church Gmail account (jubileeintf@gmail.com).

Click Blank Form and title it “Jubilee International Fellowship — Prayer Request.”

Add fields for: Name, Email (optional), Type of Request, Prayer Request, and a checkbox for Keep Private.

Click Send, then the link icon, and copy the form URL. Paste it into this page where it says YOUR_GOOGLE_FORM_LINK_HERE.

In form settings, turn on Email notifications so every submission arrives in your inbox automatically.

You can remove these instructions from the page once the form is set up.
